If you would like specific CAGIS information you can download this form, fill in the information and do one of the following:

1-Bring it to the Engineer's office on the 7th floor of the County Administration Building.  We will take your information, advise you of the cost and have it ready for you within two business days.

2-Fax it to 513-946-4288.  We will call you when it is ready.

3-Mail the form with a check (payable to:  Hamilton County Treasurer) to cover media cost plus $1.00 postage and handling to:
