Petition for Domestic Violence Civil Protection Order.
You must complete this form and file it with the domestic relations court if you want the court to issue a civil protection order. You are also required to complete Form 10.01 Z Domestic Violence Petitioner / Respondent Information Sheet. This information sheet is confidential and will only be used by the court and the Sheriff's Office in their efforts to serve the Respondent. In addition, please complete the Domestic Violence Referral Form.
You must present all three completed forms to the Domestic Relations Docket Office in room 3-46 for filing and to receive your hearing date.
