Overview
The Hamilton County Child and Family Health Services Consortium (CFHS) is a group of health care providers who have worked together for over twenty five years to impact the health status of young, uninsured women and children in Hamilton County. The Consortium identifies health needs, service gaps and barriers to care for high risk, low income families and children and then plans community public health and clinical services to meet those needs. CFHS programs are funded through a grant from the Ohio Department of Health. The CFHS grant is administered by the Hamilton County Family and Children First Council (FCFC) who contracts with providers for provision of services and programs selected by the Consortium.Meetings
The Hamilton County CFHS Consortium, which is an active committee of FCFC, is comprised of many The Consortium meets monthly to assess progress on Consortium goals, to coordinate services, to review CFHS programs, to receive current data about health care needs and gaps in Hamilton County and to exchange information. Meetings are typically held on the first Thursday of every month at 2:00 at the Hamilton County Public Health building at 250 William Howard Taft Road. Meetings are open and anyone is welcome to attend. Please call the Council office at 513-946-4990 on the first Thursday of the month to make certain a meeting is scheduled that day if you plan to attend.CFHS Grant
Every five years the Ohio Department of Health issues a competitive RFP for counties interested in impacting the priority populations and outcomes identified by ODH as being vital to the overall health of the community. The components of the CFHS grant are community health assessment and planning, child and adolescent health, prenatal health and Ohio Infant Mortality Reduction; one or more of those components had to be addressed in the grant application. The three measures/targets selected by ODH for this five year grant are: prematurity, infant mortality and overweight children.Community Health Assessment
One of the essential tools used by the Consortium to identify county needs and to look at county wide progress in improving child and family health outcomes is the Hamilton County Maternal and Child Health Assessment (MCHA). The purpose of the MCHA is to:
