When and Where?

How do I schedule my appointment?

When scheduling your appointment, please have available:

• Your full name
• Your employer (Hamilton County)
• Insurance plan name (if you’re covered under a County Humana plan – just say “Humana”)
• Date of birth
• Social security number
• Desired appointment time & location
• Name of physician to whom mammography results should be mailed

Will I have a copay?

American Cancer Society’s Mammography’s Guidelines Recommended for the Early Detection of Breast Cancer are:

Women between the ages of 20-39

• Clinical breast examination by a doctor or nurse every 3 years
• Monthly breast self-examination

Women age 40 and over

• Annual mammography
• Annual clinical breast examination by a doctor or nurse (should be done close to the time of the mammogram)
• Monthly breast self-examination
